Release of Liability for Minor Participants
Read before signing
In consideration of my child/ward ("Child") being allowed to participate in physical fitness programs with Project Blueprint, and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) to my child from the activities involved in these programs are significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,
I understand that there is an element of risk inherent in youth sports and physical fitness, and as parent/legal guardian of the child, I take full responsibility for his/her actions and physical condition.
Furthermore, I understand that certain risks, dangers, and injuries due to things such as inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to youth sports programs exist. Therefore, it is recognized that it is impossible for Project Blueprint, its employees, owners, members, agents, and assigns to guarantee absolute safety.
For myself, spouse, and child, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of child or others, and I assume full responsibility for child's participation; and,
I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from participation and bring such to the attention of Project Blueprint management immediately; and,
I myself, my spouse, child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Bryan Wolfe, and all other mentors, representatives, volunteers, instructors, agents, and Project Blueprint, LLC, its directors, officers, members, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event(s) ("Releasees"), with respect to any and all injury, illness, disability, death, or loss or damage to person or property incident to child's involvement or participation in this program, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
I, the parent/guardian, assert that I have explained to child: the risks of the activity, his/her responsibilities for adhering to the rules and regulations, and that my child understands this agreement.
I certify that the child is in good physical health and has been medically cleared to be able to undertake and engage in the range of physical activities in which the child chooses to participate.
I consent for child to be subject to random urinalysis to ensure that the child is participating in the program without additional risk to himself/herself or others.
I allow Project Blueprint to use child's image on any and all social media platforms or promotional materials.
I agree to update Project Blueprint in any change in child's physical or mental condition that could jeopardize his or her health, or the health of other participants.
This release shall include, but not be limited to, claims and damages caused by Project Blueprint transporting the child as necessary for the child to participate in its programs and activities.
I, for myself, my spouse, and child, have read this release of liability and assumption of risk agreement, fully understand its terms, understand that we have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
